Dear Friends:

Hello, my name is Jameson K. Hirsch, Ph.D., and I am a Clinical Psychologist at East Tennessee State University. I am conducting an online survey on the effects of Fibromyalgia on daily life and your feelings.

Little is known about the emotional status of individuals with Fibromyalgia; thus, your participation will help to raise awareness about Fibromyalgia and will help healthcare professionals to better understand how Fibromyalgia affects mental health.

Your participation entails filling out a short survey. Participation in this survey is completely voluntary and confidential. It will take about 30-45 minutes to complete the survey.
