Please read these directions carefully - Mapquest and GPS systems often mistakenly lead you to the Marriott Hotel which has the same address as the wellness center but is located one mile west of the center in Ronkonkoma.

Long Island Expressway (LIE, I-495) EASTBOUND: Holbrook Wellness Center is located on the LIE service road diagonally or (directly) across from Exit 61 so it is dangerous to exit here. For safety's sake, get off at Exit 60. Drive for about 1-1/2 miles. YOU MUST GO ABOUT1 MILE PAST THE MARRIOTT HOTEL Pass LAUREL STREET. Look for blue house and large lighted white sign on the right, 5000 - Holbrook Wellness Center.

(If you accidentally drive beyond the center, you have to circle around to your right at the next traffic light (Patchogue-Holbrook Road), bear right on Smith Street, turn right on Union Avenue, drive one block and turn right on Maple Avenue, drive to the end of the Maple where it intersects with the Service Road. The office parking lot is on the left side of the road).

Long Island Expressway (LIE, I-495) WESTBOUND: Drive west to Exit 61 - turn left to cross over the LIE (Patchogue-Holbrook Road), just after the new GULF station bear right on Smith Street (drive one block), then turn right on Union Avenue, (drive one block), turn right on Maple Avenue, drive to the end of Maple to just before the LIE Service Road.

The office is in a blue house, drive into the parking lot on the left side of the road.
