Usually they do back of entire site. Some hosting specifically ask you what folder you want back up.
It's a case to case thing from different hosting companies. I use hostgator and they do back-ups of entire site.

So I'm in the process of extricating myself from Dreamhost. Yesterday, based upon other users' feedback I chose WP Engine as my hosting site.
Today in an email exchange w/ the WPengine tech staff, they mentioned that they do backups but not of
the /wp-content folder.
I am shocked. Is it standard procedure for Web hosting companies not to backup
the /wp-content folders? Since my site is an Art/Design site, I have a LOT of image in that folder.
BTW: Of course, I am planning to do local backups of my site. I am just expected the hosting company to be
backing up my entire site too!
