Last evening, I mentioned "The Jet-Propelled Couch," a fascinating story about the psychotherapy of a physicist with delusions about an alien civilization. This may have been a case of paranoid schizophrenia. The epistemological aspect arises from the possibility of an internally-consistent narrative, and question of how to falsify such a narrative. In any case, I found the story quite intriguing when I first read it around 50 years ago. The first two links are the story. The third link has some speculation about the identity of the patient in the story.­­­
