Opportunities to learn from mistakes (or successes) are lost. Key knowledge is not found (or even looked for).
That it happens without staff even knowing the term - they just do it as part of doing their job.
My role is Collaboration & Knowledge Manager in Managed Programs, IT Solutions at Telstra. I am involved in a number of activities including SP2010 applications and enhancements to an existing Siebel CRM system.
