What to do about bike lanes in door zones?

Bicycle education suggests riding at least 5' from parked cars. ( http://www.youtube.com/watch?v=1TQ7aID1jHs ) Some of Portland's bike lanes (Forest, Deering between Park and Bedford) place you closer than that unless you ride very near or even outside their traffic edge. ( http://www.meetup.com/Portland-Bike-Commuting/photos/774352/#12028999 and http://www.meetup.com/Portland-Bike-Commuting/photos/774352/#12068766 ) Here are some steps that could be taken to lessen the encouragement to ride in the door zone. Which ones do you like? (If you have other ideas or want to discuss, please start a message thread about it.)
