I agree with Rick if you just want a form to do this.  But, if you want to keep in contact with these people and send them information from time to time, depending on what information they want, I suggest using a newsletter component.  I suggest using Acajoom.  You could create a personalized newletter for each question and have a link to a pdf download page on the site…and track how many people click through.  Could help with your marketing and SEO.  Also…will make you look 100% more professional.

You could probably set this up pretty easily using BreezingForms.  You might have to do a little bit of PHP scripting, but there are plenty of examples in the docs and on the forum to help you do this.  It costs $39 for a 6-month subscription up to $119 for a lifetime subscription.  Supports 1.5, 1.6 and 1.7.  I use it all the time and like it a lot.
