Impact Statement - Sailing Volunteers

Hi Group,

I am looking for some design feedback it will take about 10 minutes of 
internet browsing time.

Impact statements for nonprofits are a measuring tool for their 
effectiveness.  To simplify this process I have created an online widget 
for the entry of event information so that impact results can be 
recorded displayed in a meaningful way.

I want to design a similar database and widgets for use by our members 
to record their volunteer hours and for organizers to record their 
impact on the community.

using, I will design a separate one for Atl volunteers.  Your 
suggestions of what to include would be welcomed.
