 Bailey is almost 5 and has been doing the same thing... Bathroom rug! Would love to hear the group's thoughts ?

Today, he did it again. Three times! He was outside a lot today and had many opportunities to pee. As far as I know, it's always on the same rug (a beige sisal). Has anyone else experienced something like this? Am I not giving him enough attention? Could there be a medical issue here that the vet didn't see last time? I'd love to hear any thoughts.
