I just got back from #FILive conference
A mobile survey should be more concise than the option mentioned below. No text input. 2 minutes it's A LOT of time for a mobile survey. Besides super intrusive. Reco.: consolidate your Qs 5 max and have 3 answers max. Concentrate on the most important qs. You don't need a novel. Functionality: after user has used your app, may be at the 3rd time you could open the survey pop up. This way you know the user is actually using your app and is revisiting.

Be the user. Who would like to spend 2 minutes on a survey? While being on a mobile device.  

Consider the nature of your app. Tablet or phone? Tablet is a more calm experience than a phone app exp. 

For phone functionality:
Open survey after user revisits your app-for the 3rd time.
5 questions max
3 answers bellow qs- select answer upon tab - no text input. May be add a suggestions box at the very end. But no more.

Have empathy for the user and concentrate on the most important questions you need.
