I would say Google checks the User-Agent on requests. It recognizes that you're using an iPhone to view the page and checks to see if what you've searched for is in/relevant to the Apple Store (and if it's free, possibly).
If it is it then adds the "Free" link to the results. Tada!

Background:
When a browser requests a web-page from a server it will send along a bunch of "extra" information. (ie. The browser type, Cookies, ...)

This is a very* general explanation of what goes on, but you should get the idea.
