c) Support a variety of shipping options including real-time quotes from UPS, Fedex, USPS (so no PayPal)
d) Support a variety of common gateways (ideally including Authorize.net)
e) Be a hosted solution so it's can be more reliable and secure than my client's shared hosting plans allow (so no X-Cart, Zen-Cart, etc.)
f) Be PCI-compliant, reasonable mature, reliable, widely adopted, well-supported, etc.

The closest thing I've found is using Mals "Post Directly to Payments" (http://www.mals-e.com/tpv.php?tp=28) but it doesn't seem to support real-time shipping quotes from UPS, Fedex, etc.
