How do you feel a lack of knowledge management impacts the organisation?

Poor knowledge management leads to ad-hoc just-in-time model, with inadequate benchmarking. This results create a myriad of false metrics which erodes an organisation core business, and organisational effectiveness.

What constitutes good Knowledge Management practice in your eyes?

A reduced burden of data collection, open and free (as in speech) data sets based on agreed standards, and analytical/reporting tools that are core capability for all staff. Also open and flat communication capability and information sharing.
