It leads to short-term thinking and makes an organisation more open to unprincipled and unethical behaviour, particularly when lessons learned from past failures are not given the same weight as successes.
Creating an environment that encourages people to learn from mistakes, share their experiences freely with others and insists on senior managers being accountable for their decisions and actions to staff.
