How do you feel a lack of knowledge management impacts the organisation?

Clearly there is cost associated with repeated 're-discovery' and 're-learning' in the organisation. Also better morale when knowledge is open shared and available within the organisation.

What constitutes good Knowledge Management practice in your eyes?

Documented best practices and an environment where knowledge is shared (and not used as a powerbase by a group or an individual)
