How do you feel a lack of knowledge management impacts the organisation?

Lack of knowledge management (KM) has a lasting effect on an organisation's ability to deliver the value to customers and stakeholders. Without proper KM, information, learning from project is maintained as tacit knowledge which moves with people and as people change jobs that knowledge is lost.

What constitutes good Knowledge Management practice in your eyes?

Easy KM processes which are well supported by technology solutions that focus on development of an organisational habit (Give everybody a reason to manage knowledge. We will do it not because we have to but we want to). Lean documentation - at least capture the priority item which has a long term benefit for the team and management decision making and then if possible move on to the less priority yet necessary information.
