How do you feel a lack of knowledge management impacts the organisation?

Have seen it over and over--lack of corporate memory, left hand not knowing what the right hand is doing, and history repeating itself.

What constitutes good Knowledge Management practice in your eyes?

I'm more concerned about the major principle: all staff having easy access to what they need to know, when they need to know it.
