Talk - Goal setting - what really works?

Goal setting has been used for decades in psychotherapy, education and the business world to focus the mind to achieve specific tasks. This could be anything from verbalising an intention to achieve by the end of the day to visualising where you will be in five years, but how does it work and what works best?
