How to have truly open discussions?

There are at times folks in the group who are more talkative and assertive in stating their opinion and oftentimes dominate the discussion to the detriment of other less assertive and/or quiet members. Additionally, there is a periodic problem with 2 people attempting to talk over each other. The question, how should the organizer address this situation?
