Trello is great if you're small since it can be setup to support any
group's SDLC (as someone else mentioned).

Now that we're larger, and dispersed - we've had great luck with
FogBugz (also from Fog Creek), the FogBugz wiki and a custom PM
charting/burndown tool (that we may open source) that we built on top
of the FogBugz API.
