HNY doesn't include dental, vision, mental health,
chiropractic, ambulance or other services - it's very basic. HNY includes these
(EXCEPT ambulance) for roughly 30% more in premiums. The catch is that HNY+ is
obtained through becoming an employee of a list services company where you have
to work at least one hour per month filling out surveys or (less attractively)
stuffing envelopes. Therefore, it's faster/easier to get covered with
HNY+ than with NYS-direct HNY. Also, the HNY component
is delivered as is basic HNY - i.e., you can select Empire Blue Cross/Blue
Shield, Oxford, or other carriers.
Freelancers Union may be a better way to go, but note that
their requirements have changed: previously, a tax return with Schedule C
was sufficient, but now (I don't when this changed - sometime in the past two
years, I believe) additional proof-of-income as a freelancer is required. In
other words, if one has had a run of bad (gig-free) luck, they won't accept
the application.

I believe there are some qualifications criteria you need to meet.
