Responsive Development Processes

While most peeps are starting to understand what responsive is, or at least the basic media-query syntax, it does shake up a lot of our normal development methods and processes. Do you start with wireframes, prototypes, or PSD mockups? Where do you draw the line with adaptive and responsive logic? Does responsive design completely replace the need for mobile sites and apps?
