Agent Incident Response and Retros

Agent incidents are an entirely different beast than the average run-of-the-mill incidents, and the retros are incredibly complex. Agent involvement greatly increases the potential blast radius, and even if the underlying reason for the incident is possibly known, agent involvement can also hide and/or exacerbate the problem.

Because agents are decision-making principles in a given system, the result is that they can often take actions that would not have normally occurred within the system. In addition, in order to retro an agent incident, you must recreate the conversation thread if multiple agents are involved, and you may need to look at the thinking traces of at least one or all agents during that process.

The tooling for retroing these kinds of incidents is still not standard, but I will go through some of our agent incidents and retros as well as the Hugging Face incident and retro to bring out what may be the beginnings of agent incident best practices.
