Details
When things go wrong – the human touch; with Dawn Childs on Wed 11 September. Doors open 7:15, talk starts 8:00pm
In this presentation I will look at three case studies of significant events that I have worked through or where I have dealt with the recovery and investigation. I will review the causation of these accidents/incidents and in particular identify the human element within the chain of events to understand if they could have been prevented. I will describe the changes that have been made to avoid reoccurrence.
