How do you feel a lack of knowledge management impacts the organisation?

KM can make the difference between the organisation being merely a bunch of individuals, vs a targeted and focused team with a clear sense of identity and purpose. Interesting question vis a 'lack' of knowledge management - surely only an amoeba does not knowledge manage?: but the more complex, large, spread-out, risky/risk-averse etc the org is, the more a lack of formal KM impacts the organisation. And that needs to be done in a way that nurtures and strengthens, the wrong KM can be more toxic than 'none' at all.

What constitutes good Knowledge Management practice in your eyes?

KM that is clear and realistic about the organisation's capabilities, drivers, and environment - and actively captures and promotes the knowledge that allows it to maximise the capabilities, to satisfy the needs, and effectively adapt to the threats, opportunities etc in the environment. All the while recognising that successful organisations are full of humans, not robots (as cool as robots may be).
