After many years of working the agile and lean way, and hundreds of classes held for people responsible for product development, product-market fit, product- and service discovery, and areas like that, we still see a gaping hole in many organizations. According to the Gartner group about 50-70% of effort in this area is suspected to be wasted, as the stuff developed is something precious few want.

Sometimes people in charge do not know what tools to use to discover the facts, organize their thinking, and prioritize rationally. Sometimes they have not been given the mandate, or if so only in name, and management will at any random moment come skating in on stage and overturn decisions and plans. And finally, some organizations are simply in the old-fashioned headless chicken mode about where their customers are really at these days.
