Families in Roxbury, Dorchester, Mattapan and East Boston, consistently cannot reach the healthcare resources they need. Many of these individuals fall into lower socio-economic statuses and are struggling to feed their families while managing their own health.

By not having regular, recurring access to the care they need, these folks are left to neglect their health, which often leads to increased medical costs down the road.

From a provider perspective, doctors cannot efficiently care for the patients. They cannot emphasize the importance of proactive care, monitor the effects of prescription drugs, or get on the right track with the patients.

We have found that these patients are too busy surviving and providing for their families. This is costing everyone involved in the healthcare system precious time and resources. We have more tests, more diagnoses, more drugs, but less successful outcomes from the lack of access to these resources.

With this problem at hand affecting an increasing number of individuals, how might we deliver care outside the walls of a provider setting to families disproportionately affected by barriers to healthcare in the Boston area?
