Living While Mortal - Advanced Heath Care Planning Forms

Join us for a warm, supportive, and interactive workshop where we explore essential tools for advanced health care planning. We will clarify various documents such as the Advanced Directive, Health Care Proxy, POLST/MOLST, DNR, and other tools such as Five Wishes and the Vial of Life. With guidance and gentle encouragement, we’ll not only learn about each form but begin the meaningful process of filling them out together. Come prepared to reflect, share, and take empowered steps toward ensuring your values and wishes are known and honored. Pens, support, snacks, and heart-centered facilitation provided.

This event is brought to you in part by Ethos and LGBTQ Senior Housing. It will take place in person in the Community Room at The Pryde in Hyde Park.

The Pryde is accessible by car, commuter rail (Fairmount or Providence Line) and MBTA bus (32 or 50 bus routes).

Parking: There is free street parking on a first come first served basis. Additionally, there is a free municipal parking lot 1 block away from The Pryde Community Center. This lot is across from the Hyde Park Branch of The Boston Public Library.
