Icebreaker Matchmaking Boardgame

Details
We meet at Panera Bread, then we play an icebreaker matchmaking boardgame.
The game has 3 phases. The first phase is generating questions and takes about 10 minutes.
Players write questions that can be answered on a scale of zero and 100. For example, "On a scale of 0 to 100, how much do you like blank? How many siblings do you have? Are you a dog person, cat person, or both? (Answer 0, 1, 2, or 3.)"
Next, the Game Master collects everyone's questions in a way that anonymizes the writer, then combines them to make a questionnaire. Players answer each question. If you answer zero, it means you skip that question. You're basically filling out a Scantron, but instead of 5 bubbles, there's 101 (because there's 101 integers between zero and 100).
Finally, the Game Master collects everyone's answer sheet, does some math, and returns 2 things: 1) a similarity score between you and every other player 2) a list of questions that you and the other player both gave positive points to.
Bonus: you can tell me that you swipe right on another player, and if they tell me independently that they swipe right on you, then I'll tell you two, "It's a match."
