Agile Transformation Failures - Part I

Details
While many organizations have been trying to execute Agile transformations over the past 20 years, they have not been enough to help enterprises truly compete in the increasingly digital world.
Increasing number of Agile transformations are failing or not achieving all the objectives. We have several agile methods/frameworks, increasing number of tools/automation, passionate professionals, etc.
Why are we still failing?
In a series of meetups we will examine some reasons for failure and also encourage the participants to bring their own observations about failures.
The goal is:
What can we learn from these failures and increase our likelihood of success?
