PATH (People Assisting the Homeless) is a nonprofit organization focused on ending homelessness for individuals, families, and communities across California. PATH uses a Housing First model, meaning they prioritize getting people into permanent housing and then provide voluntary supportive services for stability. They also build affordable housing through PATH Ventures, which operates statewide. Their “Making It Home” initiative in Orange County is part of this mission and includes several programs and services:

  • Navigation Centers:
  • Placentia Navigation Center (opened in 2020) – 100 beds for North Orange County and Yale Navigation Center (opened in 2021) – 425 beds for Central Orange County.
  • Housing Programs:
  • Emergency Housing Vouchers, Rapid Re-Housing services, Housing navigation, deposits, and tenancy-sustaining support
  • Outreach & Care Coordination:
  • Street outreach and case management, Partnerships with CalOptima Health for enhanced care and community supports
  • Impact:
  • Over 2,000 people served and 659 housed since 2021 in Orange County
