Meet Kimberly Preto-Diaz of

Care Matters Always an incredible transition partner who brings expert insight and heart to the conversation.
Kimberly Diaz is a Registered Nurse with over 26 years of experience and a fierce passion for supporting

families through the complexities of aging and dementia. She is the founder of Caring Minds Dementia
Consulting and Training, Care Matters Always, and Care Matters Registry. As a dementia consultant, care navigator, and trauma-informed clinician, Kimberly empowers caregivers with the tools, knowledge, and
compassion needed to thrive—not just survive—in their caregiving journey.
Inspired by the love and strength of her own grandparents, Kimberly has dedicated her life to being a
voice for those who can no longer speak their truth. She brings clinical expertise, deep empathy, and real-world strategies to every conversation.
Today, she’ll guide us in exploring when it might be time to move to an independent care community and how to talk with your family!
