There are many faces of care coordination: care teams, patient portals, secure communications and transitions of care. Who else needs to be invited to the table? How do we make the patient front and center? What needs to emerge in the world of health services and technology to help teams identify and maintain individual patient goals, optimal pathways toward those goals, and then manage the participants toward shared success?

Please join us for an engaging conversation about how technology and services can join forces to help solve this critical issue in healthcare. Food and beverages will be provided.

Speakers:

Abbe Don - VP of UX, Epocrates: As Vice President of User Experience at Epocrates, Abbe Don is responsible for creating a contemporary, innovative end-to-end mobile customer experience for caregivers. Prior to joining Epocrates in June 2013, she spent 25 years focused on interaction design in the consumer space for companies such as Apple, The Walt Disney Company, Hewlett-Packard, and IDEO.

Lindsey Kempton - Senior Business Development Associate MDP Accelerator, athenahealth: Lindsey Kempton is a member of athenahealth’s Business Development team for the More Disruption Please program, which includes the MDP Community, athena Marketplace, and MDP Accelerator. She works with health IT companies, from startups to more established organizations, to forge partnerships that connect athena’s clients with innovative solutions. Lindsey has a background in IT research and development.

Anna Larson - CEO & Co-Founder of Hale Health, MDP Accelerator: Before launching her start-up Hale Health, Anna worked at Apple on the iPhone team managing international rollout and carrier partners. Four iPhone and two iPad launches later, Anna left Apple to start Hale Health, a new way for patients to digitally connect with their Primary Care Physicians and manage both their acute and chronic illnesses.

Alexandria (Alex) Blacker, MPH - Community Programs Coordinator, Stanford Coordinated Care: Alex has her Master’s in Public Health from San Diego State University and currently works for Stanford Coordinated Care as the Community Programs Coordinator. Coordinated Care is an innovative clinic geared towards employees with chronic health conditions. Alex leads the Disease Management Program within the clinic and works with the Hospital and University population to direct them to needed resources. A large portion of Alex’s position is to run and coordinate the Chronic Disease Self-Management Workshops. Prior to working at Stanford Alex has worked as an epidemiologist doing breast cancer research with the Cancer Prevention Institute in Fremont and as a Health Educator with the University of California, Santa Barbara.

Aaron Imholt - VP Products & Partnerships, LivWell Health: Aaron comes with15+ years in marketing, product development and professional sports in both multinational enterprise and tech start-ups such as ESPN X Games, Fluke Electronics, The North Face, NHRA & NASCAR. He drives LiveWell Health's product go to market and partner relationships and is based in the SF Bay Area.

Deuce Cruse - Senior User Experience Design Manager, Epocrates: Deuce leads the design team at athenaHealth in San Francisco that is responsible for athenaText - a mobile and web-based platform that allows easy communication across care teams while remaining HIPAA compliant. Prior to joining athenahealth in 2014, he was a project leader at IDEO focused on the design of healthcare and consumer experiences for mobile phones, desktop browsers, and other software-enabled products. He has a background in software and electrical engineering and a passion for bridging the gap between innovative design and product development. He began his career in healthcare developing patient monitoring and chronic disease management solutions for Hewlett Packard.

Moderator: Samir Panchal - Management Consultant, Freed Associates: Samir has significant experience in healthcare strategy, system selections, implementations and go-lives and revenue cycle work. Prior to his position with Freed, he was a Senior Business Development Manager with a technology firm working on the healthcare product development team for HTB which focused on healthcare mergers and acquisitions, strategy, vendor partnerships, and product development. During his career, he worked for an IT services and business consulting company in the healthcare strategy and transformation group. Samir also has two patents in telemedicine and five publications in various journals including JHIMSS and cardiology journals.
