The waiver requires an electronic signature when you reserve your spot on The Phoenix website or NewForm mobile app. Please RSVP with integrity and remove yourself from the event if your plans change to make space for someone else. While we regularly check MeetUp for messages during business hours (M-F), we are unable to respond to last minute communications occurring immediately before or during some events.
